초록 For patients with end-stage chronic kidney disease, kidney damage has already been caused. The "Chronic Kidney Disease Care and Patient Health Education Program" (Pre-ESRD program) can still be added to prevent the damage from continuing to worsen. Entering kidney dialysis through this program allows patients to have good quality of life in addition to disease control once they need dialysis treatment. Improve patient satisfaction with treatment.  1. Group health education: Pre-ESRD patients are invited every quarter; nephrologists, health care nurses, peritoneal dialysis patients or hemodialysis patients are invited to share their treatment experience.     2. Individual health education: Establish a Pre-ESRD patient health education clinic. Ask a physician to consult a health care nurse, a person to explain to the patient, and listen to the patient's wishes.     3. The multimedia player intervened, and the medical team shot a video explaining hemodialysis; peritoneal dialysis and kidney replacement, let Pre-ESRD       Patients and family members can learn about various treatment situations.     4. Health education leaflet comparing the advantages and disadvantages of hemodialysis and peritoneal dialysis.     5. Intention analysis form, making analysis forms for hemodialysis and peritoneal dialysis, allowing patients to select their own needs, assisting patients and their families to choose the dialysis method.  Through the health education system, 2019.01.01 ~ 2019.12.31, a total of 100 Pre-ESRD patients received health education: 10 patients received peritoneal dialysis treatment; 20 patients received hemodialysis treatment; 2 patients received a kidney exchange, and the awareness of dialysis 15% rose to 90%, and the satisfaction of health education reached 90%. Through systematic review, it is confirmed that a good health education will be the source of the patient's personal decision. Every Pre-ESRD patient will almost feel helpless and afraid of "dialysis". The professional and unique Educational guidance to improve the patient's awareness of the disease.
