제목	Pilot Project of Home Management in Peritoneal Dialysis Patients

초록	It is difficult to manage peritoneal dialysis (PD) patients because the medical staff cannot directly check whether they are doing well. There is still no management system including education, counseling, and patient monitoring for self-management of PD patients. To improve the quality of life of PD patients, the government has been promoted the “Pilot project of home management in PD patients” for three years since December 2019, and a total of 54 dialysis centers have participated in the pilot project. The home management pilot projects for PD patients consist of three parts: 1) educational counseling fee I (doctors provide professional and in-depth education on the characteristics of the disease, treatment methods, and management plans for dialysis patients), 2) educational counseling fee II (doctors or nurses provide educational counseling on treatment plans, prevention of complications, management on daily life and diet), 3) patient management fee (doctors or nurses provide PD management service by phone or text message at least twice a month). By April 2021, a total of 3,730 patients from 43 dialysis centers have been enrolled and 36,509 cases of medical insurance fees have been submitted. Among 3,730 patients enrolled in this pilot project, the response was particularly good for new patients. However, there are a few challenges including expansion of pilot project to the in-patients, simplification of the website registration process, and recruiting more participating dialysis centers in the pilot project. To revitalize the pilot project, the Korean Society of Nephrology has held online meetings for doctors and nurses and is trying to provide various educational materials. Hopefully, the contents and participating centers of pilot project are expanded to help PD patients maintain safe and healthy life at home.
