제목	Correlation between transient hemodialysis and bleeding complications before peritoneal dialysis catheterization

초록	Objectives: In our center, if the attending physician judges that the risk of uremic bleeding is high, hemodialysis is temporarily performed before the peritoneal dialysis catheterization. However, the effect of dialysis on platelet function and coagulation is uncertain, and there is report that hemodialysis is inferior to peritoneal dialysis in terms of platelet and coagulation. We analyze risk factors that can cause bleeding side effects in peritoneal dialysis catheterization and whether temporary hemodialysis before catheterization is helpful in reducing bleeding Methods: We retrospectively analyzed 336 patients who had a peritoneal dialysis catheter inserted between January 1,2012 and March 30, 2022. They were divided into two groups: a group that underwent temporary hemodialysis before peritoneal dialysis catheterization and another group that did not. We used χ2 test and logistic regression analysis to determine the effect of temporary hemodialysis on bleeding and risk factors. A subgroup analysis was additionally conducted for only patients who underwent hemodialysis. Statistical significance was set at P < 0.05. Results: Among 336 patients, 120 (35.7%) underwent temporary hemodialysis before peritoneal dialysis catheterization. There was no significant correlation between bleeding and transient hemodialysis (p = 0.114). In univariable analysis, there were no significant results with bleeding. However, in multivariable analysis, platelet (OR=0.99, 95% Confidence Interval 0.96-1.00, p=0.026) and International Normalized Ratio (INR) (OR=6.96, 95% CI 1.41-34.17, p=0.017) were significantly correlated with moderate bleeding. In the subgroup analysis, there were no risk factors significantly correlated with bleeding Conclusions: The results of our study did not show a significant correlation between temporary hemodialysis before peritoneal dialysis catheterization and bleeding complications. As risk factors for moderate bleeding, platelet and INR showed a significant correlation, but not in subgroups. Further research is needed to determine whether hemodialysis affects these factors.
