제목 Incidence, renal outcome, and prognosis of acute kidney injury in CKD patients with acute cholangitis

초록 Objectives: Acute cholangitis(AC) is a common infectious disease and is a very important disease that can cause mild to death. As a disease that requires early detection and antibiotic treatment, no previous studies have been conducted on the prevalence and mortality of diseases related to kidney function. Therefore, this study aims to investigate the renal outcome and prognosis according to renal function in patients with AC. Methods: A retrospective study was conducted on adult patients aged 18 years or older who were hospitalized from 2011 to 2017 with the final diagnosis of AC. Patients who were hospitalized for reasons other than AC, dialysis patients, and children were excluded from the study. To evaluate the severity of the disease, the severity was evaluated usingquick sequential organ failure assessment (qSOFA) score, and 2018 Tokyo Guidelines (TG) grade. Results: A total of 1438 patients were enrolled, of which 7.9% of patients were chronic kidney disease(CKD) patients. The normal renal function group and the CKD group were divided into renal outcomes and prognosis studies. The CKD group was older(79.3±8.5 vs. 70.2±13.2,p<0.001) and had a higher rate of comorbidities such as hypertension(54.4% vs. 33.6%, p <0.001) and diabetes (30.7% vs. 19.4%, p=0.004). AKI occurred more frequently in the CKD group(27.2% vs. 17.4%,p=0.009) than in the normal renal function group, and the 1-year mortality rate was also higher than in non-CKD group(13.2% vs. 5.7%, p=0.002). The disease severity to the 2018 cholangitis TG guideline and qSOFA (27.2% vs. 18.1%, p=0.016)were significantly higher in the CKD group. Length of stay was also significantly longer in the CKD group(13.0±9.4 vs. 10.5±9.3, p< 0.001) Conclusions: In patients with AC, CKD has an increased risk of AKI, and a poor prognosis. Therefore, clinicians should make an effort to improve the prognosis through appropriate and early treatment if CKD is present.
