제목 Post renal biopsy complications and their predictors an observational study

초록 Objectives: Introduction-Percutaneousrenal biopsyis essential for diagnosis of manyrenal diseases. Incidence of biopsy complications is 13-34% , with complications including gross haematuria, flank pain, perinephric haematoma or even death. Factors associated with these complications include older age, hypertension, more number of passes taken during the biopsy and sonological factors such as reduced cortical and parenchymal thickness, shrunken kidneys and kidneys with poor cortical echogenicity. Aim: To study the correlation of the clinical, laboratory and sonological parameters associated with the occurrence of post renal biopsy complications. Methods: This is a prospective study done from April 2022 to December 2022, on all adult patients undergoing renal biopsy- both of native and transplant kidney at Kasturba Medical College, Manipal. Baseline parameters including S.Creatinine, Hemoglobin, Hematocrit, Platelets, coagulation profile were noted. Ultrasound was done prior to the renal biopsy to assess the kidney size, cortical thickness, parenchymal thickness and cortical echogenicity. Renal biopsy was carried out under real time USG guidance and any hematoma at the time of biopsy was noted. USG was repeated at 6hr and 24hr for all patients to assess the hematoma status. Other complications such as gross hematuria, hypotension, hematocrit fall, need for blood transfusions and need for any interventions were also noted.Man Whitney U test, Chi sq test and descriptive statistics were used to analyse the data. Results: A total of 102patients were included in the study, 62.7% patients had complications post biopsy.55.8% patients had minor complications and 6.8% patients had major complications.eGFR was the single most important factor associated with the complications (Table 1). Conclusions: Overall the rate of major complications were less, with only 4.9% patients requiring an intervention. Lower eGFR was associated with higher incidence of complications.
