Despite peritoneal dialysis (PD) being a cost-effective and appealing kidney replacement therapy (KRT) option for countries with limited budgets, PD penetration accounts for only 13% of the total dialysis population, with even lower numbers in limited-resource countries. While the choice of KRT may be influenced by factors such as doctors’ preferences, health infrastructure, and financial incentives, expanding PD penetration is achievable through the following strategies: 1) Involving policymakers in the process; 2) Empowering chronic kidney disease (CKD) and kidney failure patients and caregivers to access KRT while ensuring desired outcomes within a sustainable healthcare program; 3) Ensuring fair pricing of PD solutions through central procurement or by building local capacity; 4) Establishing an informative dialysis registry database; 5) Increasing incentives for PD provision, avoiding value-based incentives, and addressing conflicts of interest; and 6) Implementing effective monitoring and auditing systems in dialysis centers. |
