초록 Recurrent glomerulonephritis after transplant by Professor Stephen Marks The aim of this session is to understand the factors involved in determining renal allograft survival in paediatric kidney transplant recipients and to consider how to improve patient and renal allograft outcomes in recurrent disease after transplantation. There will be discussion of the causes of graft dysfunction and outcome and consideration for the risk factors for poor renal allograft survival in children, including recurrent disease, primary renal disease, race, prolonged cold ischaemia time, delayed primary allograft function, HLA matching, prior transplantation, early acute rejection, young recipient age and deceased donor age above 40 years. The lecture will consider the immunological causes of chronic allograft dysfunction of late allograft rejection, non-adherence as well as recurrent and de novo glomerular disease after kidney transplantation. There will be reference to the non-immunological causes including vascular, bladder dysfunction and BK polyoma viral associated nephropathy. There will be discussions on how to diagnoses recurrent glomerulonephritis after kidney transplantation and which primary and secondary glomerulopathies as well as metabolic and systemic diseases can recur after kidney transplantation. The session will highlight some important case studies showing recurrent disease in patients with atypical haemolytic uraemic syndrome and steroid resistant nephrotic syndrome due to focal and segmental glomerulosclerosis. This session will also highlight the importance to the management of chronic kidney disease and its sequelae in those children and young people with chronic kidney disease secondary to recurrent glomerulonephritis after kidney transplantation.
