Diabetic kidney disease (DKD) exhibits a heterogeneous phenotype, and trajectory analysis reveals diverse changes in kidney function. Of particular note, certain patients demonstrate regression of albuminuria, while others show a decrease in GFR without albuminuria. Additionally, less than 5% of patients exhibit a rapid decline in GFR. Phenotyping these patient subsets is challenging with GFR and albuminuria tests alone, and histological assessment can provide assistance, aiding in the identification of combined disorders. Variations in interpretation among clinicians regarding the guidelines can lead to differing approaches to patients, resulting in a heterogeneous phenotype. Therefore, discussions among clinicians from various fields and appropriate consultation and referral are necessary. Omics can be utilized to define phenotypes and may serve as therapeutic targets. |
