Well proven success of organ transplantation program as a life saving and life transforming treatment for various organ failures had given rise to a universal state of organ shortage in the face of ever increasing number of patients in the waiting list for transplantation. While various public health measures aimed at mitigating the incidence and prevalence of new patients with end stage heart, liver, lungs and kidney failure take a long time to bring the desired outcome (e.g available effective treatments for Hepatitis B and C could lead to a possible reduction of cirrhosis and need for liver transplant), national health authorities and systems will still be expected to meet the high demands for organ transplants via ethical and legal means. Nations and locations which had consistently shown high performance in organ donation and transplatation employ various strategies and had adopted specific system ,policies including legislative mechanisms to strengthen each national organ donation and transplant program. Among others , the establishment of optimally empowered and adequately funded national, regional or local Organ Procurement Organizations (OPO) is critical and highly important to deal with various emotional ,social ,ethical ,legal and clinical issues and challenges in organ donation and transplantation. OPOs are responsibile for increasing the number of registered donors ,coordinate the donation process in deceased donor transplant program and ensure that all donated organs are used to their maximum benefits. At the local level , procurement coordinators from the OPO play vital roles to evaluate potential donors, manage and support the donors to optimize organ function before retrieval. Management of the donor by the OPO optimizes organ function through support of blood pressure, volume resuscitation, oxygenation, correction of electrolyte, and metabolic abnormalities including hormonal imbalances which occur in brain death potential donors. Through established rules, allocation of organs will be made by the OPO team which follow agreed policies and standard procedures to ensure equitable, ethical and rational distribution. Effective communication, accountability, transparency in the process and decision making are some of the important requirements to improve organ donation and transplant community performance. Improving relationships and enhancing communication are fundamental in building trust. Regular audit of the program activity and outcomes will not only provide assurance that the agreed and established rules are adhered to, but also instill confidence and promote trust in transplant program stakeholders, especially among the general public who constitute the pool of potetial donors. Having confidence that all organizations are performing procedures in a standardized fashion allows for process dependency, not person dependency, thus enabling more flexibility and system efficiency which will ensure a healthy development, growth and long-term sustainability of organ transplantation program. |
