제목 Management of Orthostatic Hypotension in Post-Kidney Transplant Patient: a Case Report

초록 Introduction: Kidney transplantation is one of the kidney replacement therapy options for patient with End Stage Kidney Disease. However, kidney transplantation also has several side effects that can affect the patient's condition. One possible side effect is orthostatic hypotension. If not properly managed, this condition can affect the condition of the allograft kidney as well as the occurrence of readmission to the hospital. Case illustration: We present a case of a 32-year-old man with CKD, hypertension (prior to kidney transplantation), and diabetes who underwent kidney transplantation. After the transplantation, the patient developed postural hypotension two weeks after the procedure (blood pressure dropped to 88/64 mmHg upon standing). Blood examination revealed an Hb level of 10 g/dL, and echocardiography showed an EF of 78%. Discussion: Orthostatic hypotension is one of the possible side effects that can occur after kidney transplantation. This condition can be caused by various factors, including the effects of immunosuppressive drugs given. Management of postural hypotension includes both of non-pharmacological and pharmacological therapy. Not all cases require treatment with medications. Pseudoephedrine is one of the medications that can be given to alleviate symptoms. Conclusion: Post-transplant orthostatic hypotension requires adequate monitoring and management. Not all cases require pharmacological therapy. Pseudoephedrine can be chosen as one of the drugs for the management of posttransplant orthostatic hypotension if needed.
