Objectives: Crescents are not rare in proliferative lupus nephritis and their presence usually is associated with worse renal outcomes. Such patients therefore mandates prompt intervention. However little is known about the clinico-pathological features in proliferative lupus nephritis with crescents. There is a paucity of Indian literature on the same as well. We therefore opted for this retrospective study where we sought for the clinico-pathological parameters in the subset of patients of lupus nephritis having crescents in their biopsy. Methods: Patients of lupus nephritis above 18 years of age undergoing ultrasound guided native kidney biopsy in the Department of Nephrology from August 2021 to December 2023 with histology showing proliferative lupus nephritis(Class 3 or Class 4 Lupus nephritis with or without overlapping Class 5 lupus nephritis) with evidence of crescents were recruited. Baseline parameters were seen from the database for the patients fulfilling the inclusion criteria. Results: We had 36 patients included in the study. Among various parameters, hypertension and 24-hour urine protein strongly correlated with crescent formation (p=0.019 and 0.002, respectively). Among histological parameters, crescent formation was more commonly seen in pure class 4 lupus nephritis compared to other classes (p=0.005). A progressive decline in eGFR was noted with increase in the percentage of crescents Conclusions: This study reflects the relationship of crescents with clinico-histological parameters in lupus nephritis. However a larger sample size would be needed before extrapolating our results. |
