제목 Quantitative analysis of the vorticity of the flow and vascular access sounds in a pseudo-vessel stenosis model

초록 Objectives: Auscultation of vascular access (VA) sounds is a common practice for managing the VA in hemodialysis patients. The characteristics of the VA sounds in the presence of stenosis may be closely linked to the flow conditions downstream of the stenosis, but a quantitative relationship has not yet been clarified. In the present study, we focused on the vorticity as a flow parameter and attempted to quantitatively determine the vorticity of the flow downstream of the stenosis and the acoustic characteristics of the VA sounds in a pseudo-vessel stenosis model. Methods: A model of pseudo-vascular stenosis was created by inserting a stenotic section into the downstream outflow of a Y-shaped connector simulating an arteriovenous anastomosis site. The stenosis ratio was varied in steps of 10% from 10% to 90%. Micro-particles were added to the water that was allowed to flow into the simulated vessel. The vorticity of the flow for each stenosis ratio was determined by capturing the movements of the particles using a high-speed camera. The characteristics of the VA sounds were evaluated using an electronic stethoscope placed 10 mm downstream of the stenotic part, and the normalized cross-correlation coefficient R was calculated. Results: The flow until a stenosis ratio of 30% was laminar, while the vorticity of the flow increased markedly with increasing stenosis ratios as the stenosis ratio exceeded 50%. The R decreased significantly with increasing stenosis ratios for stenosis ratios exceeding 50%, suggesting that the flow in the vessel was related to the acoustic characteristics of the VA sounds. Conclusions: The vorticity of the flow increased as the severity of the stenosis increased, and the R, which represents the acoustic characteristics of the VA sounds, decreased as the stenosis ratio increased. Changes in both of these characteristics became significant when the stenosis ratio exceeded 50%.
