제목	PREDICTION OF MAJOR ADVERSE KIDNEY EVENTS IN ICU PATIENTS WITH USE OF RENAL ANGINA INDEX : PROSPECTIVE STUDY FROM A DEVELOPING COUNTRY

초록	Objectives: Acute Kidney Injury( AKI) occurs in 15-60% of Intensive Care Unit (ICU) admissions and are associated with high mortality, morbidity and increased health care costs. Major Adverse Kidney Events (MAKE) is a clinically meaningful patient centred outcome for AKI. Renal Angina Index (RAI) is a validated tool for prediction of AKI in children and adolescents with recent studies in adult population. We sought to examine the incidence of MAKE by day 30 (MAKE30), its predictors and its association with RAI. Methods: Prospective cohort study conducted in the major ICUs of a tertiary care centre in India(June 2023 to December 2023). Ethical clearance obtained from Institutional Ethics Committee. Adult patients with ICU stay >48 hours were included, excluding end stage renal disease (ESRD) and renal transplant recipients. Baseline clinical and demographical data obtained on the day of admission to ICU. AKI was defined as per KDIGO guidelines. Renal Angina Index was calculated from change in creatinine and patient condition scores. The primary outcome measure was MAKE30 (defined as a composite of death, provision of RRT, or sustained loss of kidney function, at discharge or by day 30, whichever comes first). Results: Out of 251 patients recruited for the study, 176(70.1%) had an AKI during hospital stay, with stage 1 in 66(26%), stage 2 in 25(10%) and stage 3 in 85(33%). Overall MAKE30 occurred in 135(53.7%) with death in 70(27.8%), need for RRT in 79(31.4%) and persistent loss of kidney function in 57(22.7%). Serum creatinine at admission (OR 2.75, 95% CI 1.94-3.91, p <0.001) and RAI (OR 1.1, 95% CI 1.07-1.14, p <0.001) were independently associated with MAKE30. Logistic regression analysis showed a AUC of 0.727 between RAI and MAKE30 (ROC in Figure 1). Conclusions: RAI need further modification and validation for better prediction of MAKE 30
