제목 Clinical Usefulness of Kidney Biopsy in Pediatric Nephrotic Syndrome

초록 Minimal change disease (MCD) is the most common pathologic finding in pediatric nephrotic syndrome (NS), and therefore, kidney biopsy is not essential. However, in some cases, kidney biopsy is performed when the clinical course is different from that of MCD or when nephrotoxic drugs such as calcineurin inhibitors are required for treatment. We retrospectively analyzed the clinical data of 70 patients diagnosed with NS among 552 pediatric patients who underwent kidney biopsy at our hospital from 2005 to 2016. The mean age of all 70 patients was 8.4±4.6 years old, with boys accounting for 65.7% (46 patients). Among them, the ratio of steroid-sensitive NS (SSNS) and steroid-resistant NS (SRNS) was found to be 56:14. In SSNS, the reason for kidney biopsy was that the pre-use test of nephrotoxic drugs due to steroid dependency or frequent relapse was the most common with 78.6% of the total, followed by the age of the patient with 14.3%. As a result of kidney biopsy, 91% of the total were diagnosed with MCD, and 11 out of 12 patients (91.6%) who underwent kidney biopsy with the clinical course different from that of MCD, such as hypertension, age, and hepatitis B carriers were also diagnosed with MCD. Of the 56 patients with SSNS, 49 (87.5%) showed complete remission during follow-up and none progressed to CKD. On the other hand, as a result of kidney biopsy of 14 patients with SRNS, 7 patients, 50% of the total, had focal segmental glomerulosclerosis, 4 patients had MCD, and 3 patients had membranous nephropathy, and 5 patients, 36% of them, progressed to ESKD. In pediatric NS, it is considered desirable to determine kidney biopsy mainly based on the responsiveness to steroids and the use of nephrotoxic drugs.
