제목 Vancomycin-induced Acute Generalized Exanthematous Pustulosis in a Patient on Peritoneal Dialysis

초록 Acute generalized exanthematous pustulosis (AGEP) is an uncommon, reactive lesion which is characterized by an acute onset of monomorphic, small sterile pustules on an erythematous background. We present a case of vancomycin- induced AGEP in a patient on peritoneal dialysis (PD). A 39-year-old female on PD visited to out hospital for exit site discharge. Five months previously, she had been diagnosed with end-stage renal disease owing chronic glomerulonephritis. She had received PD as renal replacement therapy. The exit site infection poorly responded to ciprofloxacin. Result from the culture showed isolation of methicillin-resistant Staphylococcus epidermidis and the patient received intravenous vancomycin 1g/week. Two week later she consulted due to the presence of cutaneous rash in whole body without fever. A skin biopsy consisted with acute generalized exanthematous pustulosis. The patient discontinued vancomycin and received prednisolone (20mg/day), but the skin lesion did not improved. Viral pneumonia developed during steroid therapy and steroid was stopped. Ten days later, viral pneumonia and the skin rash regressed and were followed by desquamation.
