| 초록 | There have been only sporadic reports of membranous nephropathy (MN) evolving into acute crescentic rapidly progressive glomerulonephritis (RPGN). Here we report a case of rapidly progressive glomerulonephritis accompanied with renal vein thrombosis which developed in a patient with membranous nephropathy. A 38-year-old male with membranous nephropathy hospitalized due to right flank pain. A computed tomography scan revealed right renal vein thrombosis and creatinine level was 1.34 mg/lL. Therapeutic anticoagulation therapy was started with warfarin for renal vein thrombosis. The patient returned for follow-up after 6 days. His serum creatinine level greatly increased to 6.36 mg/dl and potassium level was 7.6 mg/dl. One session of emergent hemodilaysis and kidney biopsy was performed. A Kidney biopsy revealed acute crescentic glomerulonephritis and monthly cyclophosphamide was infused intravenously with oral prednisolone during three months. Serum creatinine level normalized to 0.93mg/dl and cyclophosphamide switched to mycophenolate. So we report a rare case of rapidly progressive glomerulonephritis with renal vein thrombosis in membranous nephropathy patient. |
