제목	Outcomes of Spousal Donor Graft in ABO Incompatible Kidney Transplantation

초록	Background: In recent era of organ shortage, spousal donor and ABO incompatible kidney transplantation (KT) are good alternatives for living related donor (LRD) KT. This study was to compare the outcomes of spousal donor in ABO compatible and ABO incompatible KT. Method: Total 326 living donor KT patients were analyzed. We compared the baseline characteristics, percentage of spousal donor and survival rate for grafts and patients between two spousal donor groups. Result: Percentage of spousal donor in ABO incompatible KT group (group1, 22 out of 56 patients, 39.3%) was higher than that of in ABO compatible KT group (group2, 62 out of 270 patients, 23%) (p=0.013). Both recipient and donor age were higher in group 2, other baseline characteristics showed no difference. 3-year rejection free survival rate was higher in group 2 (58.4 vs. 87.7%, p=0.007), but 3-year graft survival (85.2 vs. 95%, p=0.065) and patients survival rate (95.5 vs. 100%, p=0.093) were comparable between two groups. Serum creatinine level was lower at first 3 month in group 1 (1.25±0.24 vs. 1.45±0.37 mg/dL, p=0.028), but after 3 month, there was no difference of renal function in both groups till 4 years. In ABO compatible patients, there was no difference among LRD, living unrelated donor (LUD) and group2 in 3-year rejection free survival, graft survival and patients survival rate. In ABO incompatible patients, there was no LUD, and when we compared LRD and group1, rejection free survival, graft survival and patient survival rate showed no difference. Conclusion: Outcomes of spousal donor graft in ABO incompatible KT were comparable to those of ABO compatible KT in respect of graft and patients survival rate.
