| 초록 | Purpose: Mesangial IgG deposition is frequently observed in patients with IgA nephropathy (IgAN). However, the role of mesangial IgG deposition in the pathogenesis or progression of IgAN is uncertain. In this observational cohort study, we aimed to investigate clinical significance of mesangial IgG deposition in patients with IgAN. Methods: A total of 627 patients with biopsy-proven IgAN were enrolled between January 2000 and December 2013. Histological variables of the Oxford classification (Oxford-MEST) and the presence of mesangial Ig G deposition were assessed. The study end point was regard as end-stage renal disease (ESRD) and 50% reduction in estimated glomerular filtration rate (eGFR). Results: Of the 627 patients, 200 patients (31.9%) had mesangial IgG deposition. During a mean follow-up of 56.8± 37.5 months, the incidence of ESRD was also significantly higher in the IgAN patients with mesangial IgG deposition relative to the IgAN patients without mesangial IgG deposition (28.0 vs. 10.3 per 1000 patients-years; p=0.001). A Kaplan-Meier analysis revealed that cumulative renal survival was significantly lower in IgAN patients with mesangial IgG deposition (p<0.001). In addition, mesangial IgG deposition significantly predicted renal outcome of 50% decline in eGFR independent of Oxford-MEST and clinical features in a multivariate Cox proportional model (HR, 1.82; 95% CI, 1.00-3.39; p=0.049). Conclusions: This study showed that that mesangial Ig G deposition were independently associated with poor renal outcome in patient with IgAN suggesting that mesangial IgG deposition may predict renal outcomes of IgAN. |
