제목	A Case of Unilateral Acute Renal Cortical Necrosis with Ovarian Hyperstimulation Syndrome

초록	Acute renal cortical necrosis is a rare cause of acute renal failure secondary to ischemic necrosis of the renal cortex. Acute renal cortical necrosis is usually bilateral condition, rarely being unilateral. And the most common cause of acute renal cortical necrosis is obstetric complication. We describe here a case of 32-year-old woman with unilateral renal cortical necrosis with ovarian hyperstimulation syndrome (OHSS). She visited to our department with right side motor weakness and aphasia. Brain MRI showed left middle cerebral artery (MCA) territory infarction. She was preparing for in vitro fertilizatio (IVF) and had been ovum pickup 3 days ago. Laboratory findings were hematocrit 55.4 %, WBC 446,90/uL, serum creatinine 1.54 mg/dL. Abdomen sonogram showed an increased size and multiple cysts in both ovaries. For evaluation of thromboembolic complication, we performed the abdominal and pelvic computerized tomography, and acute cortical necrosis of left kidney was found. We used anti-thrombotic agent for Lt. MCA territory infarction and renal cortical necrosis. Serum creatinine level decreased to normal range (1.24) and her neurologic symptoms were improved after hospital 30 days.
