| 초록 | Xanthogranulomatous pyelonephritis (XGP) is an unusual variant of chronic pyelonephritis, characterized by destruction of renal parenchyma and replacement by lipid-laden foamy macrophages. It usually occurs in patient with recurrent urinary tract infection with or without urinary tract obstruction. However, there is no report of XGP in polycystic kidney disease (PCKD) patient. Therefore, we report extremely rare case of XGP in patient with sporadic polycystic kidney disease (PCKD). A 29-year-old woman was admitted to our hospital due to right flank pain and intermittent fever starting 15 days before. She had no underlying disease and history of recurrent urinary tract infection. Contrast- enhanced CT showed numeral cysts on liver and both kidneys and well enhancing large septated cystic mass (10 cm length) on right kidney. Open right radical nephrectomy was performed under the suspicion of RCC. The gross specimen of removed kidney showed distortion of the architecture by xanthomatous nodule and dilated pelvicocalyceal system filled with pus and blood. Microscopic finding revealed infiltration of neutrophils and lipid-laden macrophages corresponding to XGP. She is following to the outpatient clinic for eight years. The number and size of cysts from left kidney markedly increases and grows without further renal impairment. |
