Background: Simple renal cysts are common in adults but were received less attention. There was no reported study addressing the simple renal cysts arising in transplanted kidney. We investigated the new renal cysts in transplanted kidney.
Method: Computed tomography (CT) and ultrasonography were reviewed to find new onset renal cyst in kidney transplant recipients from January 2004 to December 2013. Data including age, sex, blood pressure, imaging findings, renal function, immunosuppressive drugs and other information were analyzed.
Results: We detected renal cysts in 5 of 126 (3 females and 2 males). The prevalence of new onset cysts was 3.96%, 5.45% in female patients and 2.82% in male patients. All renal lesions were Bosniak type I or II. The average duration from transplantation to detection of renal lesions was 1403 days (769-2345 days) and the mean size of cysts was 17 mm (8-28 mm). On follow up, the size was increased in one case that was not statistically significant in renal function change. Two of them showed decreased renal function at diagnosis of renal cysts and one of two was gradually progressed reduction in renal function on follow up and reached chronic kidney disease stage 5. All of decreased renal function group were female and had acute kidney injury (AKI) events more than once that were acute pyelonephritis at transplanted kidney. Four of them maintained hypertension medication and kept blood pressure under control. The mean recipient age was 46 yr-old (range 29-66) and the mean donor age was 43.75 yr-old (range 31-53). Immunosuppressive drugs were the combination of tacrolimus, Mycophenolate mofetil and prednisone in all cases.
Conclusion: We diagnosed five cases of new cyst in transplanted kidney. The prevalence of new onset cysts was 4.06% and female to male ratio was 1.94. Data did not significantly show the relationship between renal cyst and renal function. The pathophysiology and effects on transplanted kidney was undetermined and is an important question that requires further study |
