| 초록 | Background: Complications associated with ovary were reported occasionally in patients with end stage renal disease (ESRD) after initiation of continuous peritoneal dialysis (CAPD). The causes of complication were ovarian rupture, malignancy of ovary and so on. However, there were rare cases of hypotension caused by huge ovarian cyst. We report a case of 71-year-old female treated by laparoscopic removal of huge ovarian cyst, which had been grown after CAPD initiation. Methods: . Results: A 71-years old female visited outpatient department due to hypotension, anorexia and abdominal discomfort who had been receiving peritoneal dialysis therapy for 8 months. Sometimes her systolic blood pressures were checked lower than 90 mmHg, but it were recovered to normal range after position change. A huge cystic mass was found in her abdominal CT scan. Which was not checked at the time of CAPD catheter insertion by abdominal ultrasonography. It was 20 x 15 x 23 cm size well defined round contour non enhancing homogenous cystic mass in abdomen and pelvic cavity with several thin septa. We decided to perform laparoscopic cystectomy without CAPD catheter removal. And we had close observation about her postoperative blood pressure change. After laparoscopic cystic mass removal, her blood pressure measurements did not show the variation with positional change. In addition, she recovered her bowel passage and her anorexia was disappeared. The pathology of removed cystic mass was serous cystadenoma of ovary with 0.1cm the cyst wall thickness. The patient has been treated follow-up care in outpatient department for 4 months. Conclusion: . Figures: |
