제목 Can Urinary [TIMP2][IGFBP-7] predict acute kidney injury after femur neck fracture reduction operation?(pilot study)

초록 Objectives : Management of femur neck fracture is quite challenging. Various complication can be accompanied during post operation period. As a nephrologist, we focused on renal complication, especially acute kidney injury(AKI). The purpose of this study is whether the AKI biomarker, urinary [TIMP2][IGFBP-7] can detect AKI early in femur neck fracture operation. Methods : A total of 100 patients with femoral neck fracture were enrolled in this prospective observational study since May 2016. We collected patients’ serum and urine sample right after the operation (time 0). The urinary [TIMP2][IGFBP-7] were measured by ELISA method. AKI was defined based on KDIGO stage. This study was approved by Korea university institute of review board (IRB number ED 15335) Results : AKI occurred in 27(27%) patient. None of them experienced renal replacement therapy or progress to end stage renal disease (ESRD). AKI group was significantly older, showed lower hemoglobin (Hb) level and lower serum albumin at initial hospitalization time. They also showed higher blood urine nitrogen level (BUN), erythrocyte sedimentation rate(ESR), uric acid. The urine AKI biomarkers did not show significance. In multivariate logistic regression analysis, only high ESR and uric acid can predict AKI. Conclusions : This is a pilot study to evaluate the utility of urinary [TIMP2][IGFBP-7] in femur neck fracture patients. Only urinary TIMP2 showed slight tendency to predict AKI in those specific patients but there were no statistical significance. We found that ESR and uric acid elevation can predict AKI in femur neck fracture patients.
