초록	Objectives : The pathogenesis of systemic lupus erythematosus (SLE) has been attributed to complex interactions between genetic, hormonal and environmental factors. The influence of a genetic predisposing to SLE is supported by family aggregation and a high concordance rate in monozygotic twins. Methods : Here we present a rare case of lupus nephritis in identical twin sister. Results : A 32-year-old female without a past medical history, presented to the outpatient department with one month of generalized edema, febrile sensation. On physical examination, we observed fever, low leg pitting edema. Urine and blood analysis revealed proteinuria with microscopic hematuria, hypoalbuminemia and hypercholesterolemia. Renal biopsy was performed for nephrotic syndrome. She was diagnosed with lupus nephritis class V. Treatment with glucocorticoid lead to control of her symptoms and lower the disease activity. After discharged, the patient was maintained on medical therapy without new episodes of lupus flare up. Unusual point is that the twin sister of patient is diagnosed with lupus nephritis class V three years ago. Conclusions : The patient was treated with glucocorticoid, mycophenolate, hydroxychloroquine and the disease are well controlled.
