제목 The Clinical Significances of Disability in Maintenance Dialysis Patients

초록 Objectives : Maintenance dialysis patients have a high burden of disability. Few studies have demonstrated an association between disability and clinical outcomes in Korean populations. We investigated the significance of disability as predictors of clinical outcomes in maintenance dialysis patients. Methods : We recruited study subjects who were on dialysis for at least 6 months and aged 20 years and older from 27 dialysis centres (n = 1616). We evaluated the disability by 4 questions regarding activities of daily living (ADLs) to participants. Disability was defined as the inability to perform at least one of the 4 domains of ADLs with no help. Blood chemistry and demographic data were also collected for analysis. Results : The number of participants with disability was 304 (18.8%). The numbers of participants with disability were in 195 in HD patients and 109 in PD patients, respectively. The proportions of participants with history of fall during the last 1 year were 32.2% in patients with disability and 14.3% in patients without disability, respectively (P < 0.001). In univariate and multivariate analyses, physical, mental, and kidney-disease specific component scales were greater in patients without disability than in patients with disability (P < 0.001). Cox regression analyses showed that the presence of disability was positively associated with patient survival and first hospitalization free survival. Conclusions : The presence of disability was associated with fall, lower level of health related quality of life, poor patient survival. We may pay more attention to disability in the management of maintenance dialysis patients.
