Infection is the major cause of morbidity and mortality in kidney transplant recipients, particularly in Asian country. Understanding the basic immunology can guide management for our patients. Since, the standard immunosuppressive drugs are mainly target on T-cells, cell mediated immunity (CMI)-related infectious agents should be concerned. However, we are now expanding kidney transplant for reaching out our patients. There are higher numbers of recipient underwent desensitization for kidney transplantation e.g.
ABO incompatible and HLA incompatible. This desensitization process mainly removes immunoglobulin. The humoral mediated immunity (HMI)-related infection has to be concerned in these high risk patients. Balancing the degree of immunosuppression and awareness of infection, especially in the first 6 months post-operation, can leads to the excellence in long-term outcomes. |
