제목 Post Intensive Care Syndrome in post renal transplant patients in a developing nation.

초록 Aims and Objectives: Post-intensive care syndrome is a recently recognized entity defined as new or worsening cognitive, psychiatric, or physical function after a critical illness. There has been no study about the spectrum of PICS in post renal transplants. Here we address the impact of PICS in kidney transplants . Methodology: This was a prospective single center study conducted  in post transplant  ICU survivors from 2018 to 2019. All post renal transplant ICU survivors were assessed for the criteria of PICS. Posttraumatic stress syndrome 10-questions and Beck Depression Inventory II  were used for assessing psychiatric functions, MoCA scores for cognitive functions and  Hand grip dynamometer with  6m Walk distance were used  for assessing physical ability.They were screened at discharge and follow up. Readmission & mortality were compared with patients  who didn't develop PICS.  Results: 51(47%) of 108 ICU survivors developed PICS at discharge and 23(21%) were diagnosed PICS at 3 month followup. Live related transplants, high socioeconomic status, history of antirejection,Graft loss, were found statistically significant with PICS. Duration of stay and mechanical ventilation was not associated with PICS. Readmission (n =40), and mortaltiy(n = 10)  with PICS was again statistically significant when compared to control. Conclusion: PICS is a relatively new terminology in post transplants, but its incidence in our study is high, and its impact in terms of readmission and mortality cannot be overlooked. We recommend measures to prevent & treat PICS not only during ICU stay but also in followup. Cognitive behaviour therapy and physical rehabilitation which are often overlooked should be strengthened.
