제목 Outcomes of endovascular treatment by interventional nephrology fellows in an academic training program

초록 The aim of the current study is to report the endovascular treatment outcomes of an academic training program for interventional nephrology in a single tertiary care center of Korea and to assess the effectiveness and safety of vascular access procedures performed by interventional nephrology fellows in the program. Two nephrology fellows performed endovascular treatments such as tunneled dialysis catheter(TDC) placement, percutaneous angioplasty(PTA), and percutaneous thrombectomy for arteriovenous fistula(AVF) and arteriovenous graft(AVG) in the academic training program for interventional nephrology fellowship of Hallym University Sacred Heart Hospital. All of the interventional treatments were supervised during procedures by an experienced interventional nephrologist and the outcomes were reviewed via post-procedure assessment. The outcomes of the vascular access procedures were retrospectively analyzed. From January 2019 to December 2019, 524 consecutive endovascular procedures were performed on 336 patients with maintenance hemodialysis. The mean age of patients was 66.3 ± 13.1, and male patients were 52.1% (175/336). The overall success rate and complication rate was 98.7% and 1.2%, respectively. The clinical success rate of TDC placement, PTA for AVF stenosis, PTA for AVG stenosis, thrombectomy for AVF thrombosis, and thrombectomy for AVG thrombosis was 100.0%  (186/186), 98.2% (110/112), 100.0% (69/69), 100.0% (22/22), and 96.3% (130/135), respectively. There were no major complications, and all complications were treated successfully during the procedures. The outcome of the two nephrology fellows was comparable to that of previous reports. The results of the current study present that nephrology fellows are able to conduct endovascular treatments effectively and safely through the academic training program for interventional nephrology fellowship in Korea.
